Volunteer Waiver Form
All volunteers are required to read and sign a Volunteer Waiver Form at the worksite, prior to starting any work on a project.  If you like, you can print out, sign, and bring completed form to worksite.  Please do NOT mail or fax to our office.

Minor Volunteer Waiver Form
All minors (14 to 17 years of age) must bring completed Minor Volunteer Waiver Form, signed by guardian, to worksite.  No completed form, no work on project.
